Release of Claims & Fair Entry Form

RELEASE OF CLAIMS                                                     (please print portrait)

            WHEREAS the undersigned wishes to participate in a livestock show on the Richland County, Ohio Fairgrounds, and WHEREAS the undersigned knows of the perilous nature of his undertaking as it relates to injury, damage, or loss to person, life, limb and/or property,

THEREFORE IT IS AGREED as follows:

In consideration of being allowed to enter the premises of the Richland County Fairgrounds, and in further consideration of being allowed to participate in any showing, competition, performance, contest event, or any other activity involving livestock (i.e. Equine, Bovine, Swine, Canine), or involving any other kids of animals, the receipt of said consideration which is hereby acknowledged, the undersigned hereby voluntarily assumes all risks of accident or damage to his person or property; and further releases the Richland County Agriculture Society, Richland County Junior Fair Board, The Ohio State University Extension-Richland County, Barn Raisers Organization and Show Committee, Livestock Sale Committee, and their agents, officers, servants, contractors, and employees, from every claim, liability, or demand of any kind for or on account of any personal injury or property damage or loss of any kind sustained, whether caused by the negligence of any of the above entities or person or otherwise.

In signing the foregoing Release, I certify that I have read and fully understand the same, and that said Release is entered into freely and with full knowledge of its legal effect.

Note:  This Release shall be binding upon my heirs, administrators, executors, and assigns.
